Running late?
Call us at (630) 359-0105 and we'll do our best to accommodate. Appointments cancelled with less than 24 hours' notice may incur a fee.
- Photo ID (driver's license or state ID)
- Dental insurance card (if applicable)
- List of current medications and dosages
- Previous dental X-rays if you have them
- Medical history form (complete online or at the office)
- Payment method for any estimated co-pay

We accept most major PPO dental insurance plans and file claims on your behalf. No insurance? Ask about our fee schedule and payment options.
Not seeing your plan? Call us and we verify benefits for all patients before treatment begins. Out-of-network patients are also welcome.
